Necessary Affiliate Forms When Working with KGA

Welcome to the KGA EAP Affiliate Network. By gaining access to this portion of our site, you'll find the documentation you need if you have an approaching EAP Session with one of our clients, documentation to update your status as a current EAP affiliate, or documentation for becoming an EAP affiliate.

Updating EAP Affiliate Information

Please note if your affiliate information, tax forms (W-9) or confidentiality agreement is not up to date with KGA please download the appropriate documents and return them immediately to KGA. If you are unsure if your information needs to be updated, please contact Jack Burke at (508)879-2093.

Joining the KGA, Inc. EAP Affiliate Network

KGA works with affiliates from all over the country. Depending on your location, we may or may not need an additional affiliate for our network. Please do not fill out any of the affiliation forms without first contacting KGA.
